Revelation 5:2,3

Revelation 5:2 And I saw a strong angel proclaiming with a loud voice, "Who is worthy to open the scroll and break its seals?"
Revelation 5:3 And no one in heaven or on earth or under the earth was able to open the scroll or to look into it,

John is in heaven as spectator and reporter watching the scene take place.  The conclusion to the angel’s proclamation was no one in Heaven or on earth was worthy to take possession and open the seals.  

A strong angel
This is the first of three "strong angels" introduced in the Revelation. Revelation 10:1 introduces a second "strong angel" in connection with the second scroll of the Revelation. The third and final "strong angel" appears in Revelation 18:21 as the prophet of doom for Babylon, the great city. These angels are not identified by name, but by duty. They proclaim God’s revelation.

Proclaiming with a loud voice :
Why here the angle proclaiming is, the message should reach to three kind of people it is mention in the next verse
1. In heaven,
2. On earth and
3. Under the earth.

Who is worthy to open the scroll and break its seals? :
   This rhetorical question focuses on the qualifications for the task.
Revelation 5:3 And no one in heaven or on earth or under the earth was able to open the scroll or to look into it,

In Heaven
Who are all there in heaven pictured by John, four living creature, twenty four elders, cherubims, seraphim, voice of many angels, numbering myriads of myriads and thousands of thousands(Rev. 5:11),

On Earth : All creation of God,  living and non-living creature.

Under the earth :
Who are all past from the earth and living in the lower paradise and even include the Lucifer group.

No one worthy to open
The angel was looking for someone who was worthy to open the book and read it but no natural seed of man could be found in the universe to open the book. This is because all have sinned and no man is worthy (Rom 3:23). 
Even the strong angel would not open the book angels were not “Kinsman” to humanity.  because it had to be a kinsman redeemer descended from Adam.
After Adam sinned, he was no longer worthy to manage the earth. The earth was placed under the administration of the prince of the angels (Satan). He will be in authority over the earth until a kinsman redeemer is found who fulfills the requirement to redeem it.

After Adam sinned God looking who is the worthy in earth
2 God looks down from heaven upon the children of men, To see if there are any who understand, who seek God.  3 Every one of them has turned aside; They have together become corrupt; There is none who does good, No, not one. Psalm 53;2-3

Because everybody in bondage of sin in Rom 3:23 says
Romans 3:23 for all have sinned and fall short of the glory of God,

God Himself rescuer
God saw humanities situation was without hope and there was no one who could intercede on the behalf of man, therefore God Himself would rescue man. Isaiah writes about the bankrupt state of humanity in Isaiah 59.
God Himself comes to man’s rescue, being called the Arm of the Lord (Psalm 98:1,Isaiah 40:10, 51:9,52:10,53:1, 59:16)
59:16 He saw that there was no man, And wondered that there was no intercessor; Therefore His own arm brought salvation for Him; And His own righteousness, it sustained Him. Isaiah 59:16
